We are seeking an experienced Care Manager to support a caseload (approximately 6 clients few 24/7 care clients) and serve as a key point of coordination between families, caregivers, and the Avie Care leadership team.
Role Overview
This is a flexible, remote, part-time role with no fixed daily hours but 30 hours per pay period. Work is outcome-based and ideal for someone experienced in the home care, caregiving, or care management space who values autonomy and meaningful work.
You will help ensure smooth caregiver communication, proactive client updates, and organized care coordination.
Key Responsibilities
Caregiver Coordination & Communication
Serve as a liaison between caregivers and Avie Care
Support caregiver onboarding, communication, and scheduling coordination
All caregiver recruitment and screening as needed
Troubleshoot caregiver or care-related concerns with professionalism and empathy
Client & Family Communication
Act as a consistent point of contact for assigned clients/families
Provide timely updates, follow-ups, and check-ins
Ensure families feel informed, supported, and heard
Care Oversight & Reporting
Prepare monthly client updates and care summaries
Assist with compiling health updates and care reports
Track care notes, changes, and escalations accurately
Administrative & Coordination Support
Maintain organized documentation and records
Communicate regularly with Avie Care leadership
Support special care projects or escalations as needed
Qualifications
Background in home care, caregiving, care management, social work, nursing support, or health services
Strong communication skills (written and verbal)
Highly organized and reliable
Comfortable working independently in a remote environment
Empathetic, calm, and solutions-oriented
Experience coordinating caregivers or working with families strongly preferred
Role Details
Pay: $35/hour
Location: Remote (PST time zone strongly preferred)
Schedule: Flexible, no set hours (workload varies week to week)
Caseload: 6-7 clients
